      The Licensee was out of compliance with R430-90-19(13) by having strangulation hazards on stationary play equipment. In the outdoor area, there were s-hooks on the bottom of the swing with openings pointing out and up that were bigger than .04 inches in diameter. This was a repeat rule noncompliance
