      Observed through a video of the incident, that staff member one (SM1) greeted a child and an adult entering the classroom during nap time. SM1 was standing next to child one’s (CH1’s) head when a child entering the room stepped on SM1’s foot and the napping CH1's head when moving past SM1 and into the classroom which went unnoticed at the time by SM1 and the adult. Observed through video on the same day as the previous incident, that staff members were stepping over CH1 and the nap mat when a cart was present in the walkway of the classroom near the napping CH1.
