      Through staff interviews it was determined that the injury occurred on 9/5/2025 and staff were made aware by the parent that CH1 received medical attention the same day. The injury was not reported to the department until 5/20/2026.

      During the course of the inspection licensing specialist was on the playground and observed a group of eleven (11) children with one (1) staff member. Three (3) to four (4) children were playing behind a fence in a heavily treed & shrubbed area that was not easily supervised unless the staff member was standing in the trees with the children. The staff member on the playground would not have been able to see the children from where she was standing on the blacktop.
