On the ECE Playground, observed the right part of the left swing (facing the street) containing an "S" hook at the top connecting the swing with an "S" hook that has a gap larger than a dime, causing a potential safety hazard.

During outdoor walk-through observed two separate areas of the playground with tripping hazards. Both areas had multiple tree root growth measuring approximately 6 inches in height posing a tripping hazard. In addition there was a piece of metal approximately 6 feet long, measuring approximately 3-6 inches in height in various areas that protruded out of the ground posing a tripping hazard.
