In classroom 102 there was a bottle of sunscreen labeled "keep out of reach of children" sitting on a shelf accessible to children.

At the time of the visit licensing specialist walked into classroom 103 and more than half of the children still wide awake on their mats. There were 19 preschool age children to 1 staff member.

Staff member #1 was outside supervising Ch1 at the time of the injury on 2/13/2026. A staff file review was completed and staff member # 1 had an expired Full CPR and First Aid certificate and was lacking a CPR and First Aid Module on 2/13/2026.

In the hallway there were two backpacks sitting on the ground in from of classroom 112 and 113. The backpacks had a side pocket and both pockets had a container of hand sanitizer labeled "keep out of reach of children" accessible to children.

Outside on the ece playground under the slide there was less than 2 inches of wood chips. Also, outside on the fire truck playground under the slides there was less than 2 inches of wood chips

Licensing Specialist completed a safety walk through of the gym and the gym was lacking a capacity and staff- to-child ratio posting. This is a direct violation of stipulation B. Licensee shall comply with required staff-to-child ratios at all times. Ratios and room capacities must be posted in each classroom.

In classroom 109 there was a large bottle of sunscreen labeled 'Keep out of reach of children" on a shelf that was accessible. The time was very close to drop off and all the children were sitting down on the rug.

Licensing Specialist walked into classroom 127 and staff member 1 was inside the classroom with 1 child and the Para was outside on the playground with 13 children ages 3 to 5 years old. Outside on the playground the teacher was over by 3 children. Confirmed through staff members statement the para was outside alone with 13 children for approximately 5 minutes.

In classroom 127 there were 17 children present and one child was missing the time the child arrived to the center on the attendacnce sheet.

Confirmed through staff member's statements and attendance verification staff member 1 left staff member 2 with 13 children to take Ch1 to the front office. Staff member 1 was over ratio by 1 child for approximately 5 minutes.

Confirmed through staff members statement at the time if the visit the Para Haydee R walked Ch1 to the nurse's office. After reviewing Haydee's file Haydee is not a qualified Early Childhood Teacher.
