      Specialist asked staff member in Room 120 how many children they had, staff member stated 18, double checked daily attendance then said 17, then specialist counted 16 children and staff member confirmed that 16 children were present. Specialist identified that no periodic attendance verification had been taken so far during the day.

      Exposed weed matting and a small tree stump in the woodchips in front of the stage on the Preschool playground. Identified through staff member statement that Ch1 tripped on the weed matting and tree stump resulting in injury to head requiring medical attention.
