Based on observations at 10:30 a.m., S1 failed to ensure the manufacturer's restraint device was appropriate, as C1, 6-weeks-old, was too small to be in the swing. This was corrected. Corrective Action: Effective 9/4/2025, S1 stated she will refrain from putting infants who are too small to be restrained in any apparatus or equipment to ensure compliance with this regulation.

Based on observations at 10:30 a.m., S1 failed to ensure written authorization from a physician was available for C1, 6-weeks-old, as required for an infant to sleep in a car seat or other similar device, as the Specialist observed C1 asleep in a swing. This was corrected. Corrective Action: Effective 9/4/2025, S1 stated she will rock the infants to sleep and then place them in their cribs to ensure compliance with this regulation.

Based on observations and interview at 10:30 a.m., S1 failed to ensure a safety approved crib was available for each infant, as 4 infants were present, but only 3 cribs were available. Per S1, a fourth crib was available, but had not been put together. This was not corrected prior to the Specialist's departure. Corrective Action: Effective 9/4/2025, S1 stated she will ensure a crib is available for each infant present to ensure compliance with this regulation.

Based on observations and interviews at 11:00 a.m., S1 failed to ensure the center's daily attendance records for visitors included the date of visit, departure times, and/or purpose of the visit for 2 of the 5 entries. This was corrected while Specialist was present. Corrective Action: Effective 8/7/2025, S1 stated she will check the log at the end of the day to make sure all entries are complete and to ensure compliance with this regulation.

Based on observations and interview at 11:00 a.m., S1 failed to meet the required child to staff ratio for children of the following ages: 6-months to 1-year-old. S2 was observed supervising 7 children ages 6-months to 3-years-old, while S1 was preparing lunch in the kitchen. This was corrected prior to the Specialist's departure. Corrective Action: Effective 8/7/2025, S1 stated she will have a conversation with the owner who is in the process of hiring additional staff to meet the minimum staff ratios and to ensure compliance with this regulation.

B. Based on observations and interview at 11:00 a.m., S1 failed to ensure S2 completed the LDE Key Orientation Training Module 1 and DCFS Online Mandated Reporter Training prior to having sole responsibility for any children. S2 was observed being solely responsible for 3 children ages 11-months to 3-years-old. This was corrected prior to the Specialist's departure when S1 combined the two classes. Based on observations and interview at 11:00 a.m., S1 failed to ensure S2 obtained current certification in pediatric first aid and CPR within 90 calendar days from the date of hire and prior to having sole responsibility for any children. S2 was observed supervising 3 children ages 11-months to 3-years-old. This was corrected when S1 combined the two classes. Corrective Action: Effective 8/7/2025, S1 stated new staff will be supervised until they complete the CPR and First Aid training to ensure compliance with this regulation.

Based on record review and interview at 11:00 a.m., S1 failed to ensure at least two staff members were trained in medication administration whether the early learning center administers medication or not. S1 had this training; S2 did not. This could not be corrected prior to the Specialist's departure. Corrective Action: Effective 8/7/2025, S1 stated she will ensure at least 2 staff have medication training to ensure compliance with this regulation.

Based on observations at 11:00 a.m., the specialist observed C1, 6-months-old, in a car seat for 80 consecutive minutes. This was corrected prior to the Specialist's departure. Corrective Action: Effective 8/7/2025, S1 stated she will make sure no child is in a carrier for more than 30 consecutive minutes to ensure compliance with this regulation.

C.: Based on record review/interview at 10:45 a.m., S1 failed to have required documentation of current commercial liability insurance for the operation of the center to ensure medical coverage for children in the event of accident or injury. This was not corrected prior to the Specialist departure.

Based on record review at 10:30 a.m., S1 failed to have a qualified Director who is an on-site full time staff person at the center during the day time hours of operation (prior to 9:00 p.m.) and responsible for planning, managing, and controlling the center's daily activities, as well as responding to parental concerns and ensuring that minimum licensing requirements are met. This was not corrected prior to the Specialist departure.

C. Based on observations at 10:30 a.m., S1 failed to have at least two staff members trained in medication administration whether the early learning center administers medication or not. This was not corrected prior to Specialist departure.

Based on observation at 11:07 a.m., S1 failed to ensure there is a functional, readily available telephone capable of incoming and outgoing calls at all times at the center. This was not corrected prior to the Specialist departure.
