C2, five-years-old, and C3, five-years-old, were also present during the altercation.       C.: Based on record review/interview on 12/4/2024, S1 failed to notify the department and Child Welfare within 24 hours of the following critical incident: On 9/17/2024, O1 (C1's father), submitted a complaint to S1, which stated C1 was physically disciplined by his teacher. The complaint form states the incident occurred on 9/16/2024, but was not reported to the department or Child Welfare. S1 stated she did not know the incident had to be reported to the department.
