<![CDATA[<p>1711.D.: Based on observations and interview on 12/8/2025, S1 failed to meet the required child to staff ratio for children in the two-years-old classroom. S4 was observed working in the classroom alone with 11 children, aged two-years-old. S1 stated she did not realize S4 was not in ratio.</p> Corrective Action: <![CDATA[ Effective 12/8/2025, S1 stated she has hired additional staff to start working soon which will help her to ensure compliance with this regulation </p>

<![CDATA[<p>1811.A.: Based on record review and interview on 12/8/2025, S1 failed to provide documentation which showed a CCCBC-based determination of eligibility for childcare purposes from the department was conducted for S2 at least once during a five-year period. S2's CCCBC expired on 10/23/2025. S2 was working as staff-in-charge when the specialist arrived to the center, and she also assisted S5 in the infant classroom.
