1901.J.&K.: Based on observation at 9:45 a.m., S1 failed to have items that can be harmful to children kept in a locked cabinet or other secure place that ensured they are inaccessible to children. The Specialist observed a can of Lysol spray and a two bottles of chemicals in the three and four-year-old classroom under a sink accessible to the children. S1 removed the chemicals prior to the Specialist's departure. 1727.A. Based on record review at 11:45 a.m., S1 failed to provide documentation that all staff completed the online Child Abuse and Neglect Mandated Reporter Training provided by DCFS. The Specialist observed 2 out of 13 staff members did not have Mandated Reporter training. S6 (Expired: 10/4/2025), and S13 (Expired: 8/16/2025), did not have Mandated Reporter training. Corrective Action: Effective 1/27/2026, S1 stated she will update spreadsheet, already using to keep track with all required trainings, to ensure compliance with this regulation.

A CCCBC-based determination of eligibility for child care purposes from the Department was not obtained for independent contractor, prior to the person performing services at the center. The Specialist observed O3 signed into the center on 1/27/2026. The Center did not have a CCCBC-based determination of eligibility for O3. Corrective Action: Effective 1/27/2026, S1 stated she will contact Williamson therapy and any other contractor companies, for all identifiable information for each therapists, prior to entering the center, to ensure compliance with this regulation.

Based on record observation at 11:45 a.m., S12 failed to have items that can be harmful to children kept in a locked cabinet or other secure place that ensured they are inaccessible to children. The Specialist observed a stapler on the counter accessible to the children in the three-year-old classroom. S12 removed the stapler prior to the Specialist's departure. Corrective Action: Effective 08/26/2025, S1 stated she will monitor the classrooms to ensure harmful items are out of the reach of the children to ensure compliance with this regulation.

1103. A.4.5.C.1.2.: Based on record review and interview on 06/18/2025 at 10:30 a.m., S1 failed to notify DCFS and the Department within 24 hours of a Critical Incident. On 6/4/2025 at approximately 5:00 p.m., C1's mother, S17, advised S1 that C1 (3-years-old) said his teacher, S18 hit him. On the morning of 6/5/2025, S2, S17, and C1's father reviewed the center's camera footage for 6/4/2025. At approximately 9:45 a.m., S18 (DOH 5/15/2024) was observed using profane language towards C1, then walked over to him and hit him in the back. S1 reviewed the camera footage on the afternoon of…

1509.A.8.ab.Based on record review and interviews on 6/18/2025 at 11:00 a.m., although the center has implemented a Behavior Management Policy that prohibits physical punishment and verbal abuse, on 6/4/2025, at about 9:45 a.m., S18 (DOH 05/15/2024) was observed on video footage using a prohibited method of behavior management. S18 walks toward C1 (3-year-old), subjected him to verbal abuse by using profane language towards C1, then hits him on the back. S18 was terminated on 06/05/2025. Based on observations/interviews at 12:00 p.m. on 3/6/2025, S8 failed to supervise children at all times and know the whereabouts of children, as on 2/14/2025, at 5:05 p.m., C1, a one-year-old, was left unsupervised on the playground for approximately one minute. S3 looked out of the infant classroom window to the playground, and observed C1 in a playhouse on the playground playing unsupervised. She opened her classroom door and directed C1 inside. S3 then notified S2, who took C1 to his classroom with S8.

Based on record review/interviews at 12:00 p.m. on 1/30/2025, S12 failed to supervise children at all times and know the whereabouts of children, as on 1/16/2025, at 11:32 a.m., C1, a one-year-old, was left unsupervised on the playground for approximately 20 seconds. S8 looked out of the window to the playground, and observed C1 sitting in a car playing unsupervised. She informed S1. S1 then went to the playground and brought C1 inside to her classroom.
