Based on record review at 11:45 a.m., S1 failed to have a CCCBC-based determination of eligibility for child care purposes from the Department for S14 (DOH 10/01/2025) on the center’s CCCBC roster at all times. Corrective Action: Effective 12/17/2025, S1 stated she will ensure all staff have a CCCBC on the centers roaster by checking monthly to ensure compliance with this regulation.

1911.G. Based on observations at 9:54 a.m.,S1 failed to ensure pacifiers were not attached to the infants clothing. The Specialist observed a pacifier attached, by a clip, to the clothing of C1, age 12 -months-old. S26 removed the clip during the licensing visit. Corrective Action: Effective 12/17/2025, S1 stated she will remind staff to not clip pacifiers to infants clothing to ensure compliance with this regulation.

1103.A.5.C.1. Based on interviews at 12:00 p.m., S1 failed to notify LDOE within 24 hours of the following critical incident: On an unknown date and time, S1 stated DCFS visited the center to investigate an allegation that on 4/30/2025, during morning drop-off (time unknown), the children were noticed in the classroom barricaded onto a rug by classroom furniture and a stroller, preventing the children from moving. On an unknown date, at an unknown time, DCFS visited the center but failed to sign in and out on the daily attendance log. Corrective Action: Effective 7/14/2025, S1 stated she will have all visitor sign in and out on the daily attendance log to ensure compliance with this regulation.

Based on record review/interviews on 4/3/2025 at 2:10 p.m., S1 failed to notify DCFS within 24 hours of the following critical incident: On 3/3/2025 at 4:51 p.m. O2 reported to S1, through Procare, that O1 observed C1, age 4, unsupervised in his classroom when he picked him up for the day. Corrective Action:

Based on record review on 4/3/2025 at 11:00 a.m. S1 failed to ensure the computer sign in/out procedure for the daily attendance log for children included the time of departure of each child and the name of the person to whom the child was released as 6 of 92 failed to have time of departure and first and last name of whom the child was released on 3/3/25. Corrective Action: Effective 4/4/2025, S1 stated she will verbally remind parents every day and review the sign out sheet prior to the child leaving the center for the day to ensure compliance with this regulation.

Based on record review and interview on 4/3/2025 at 2:00 p.m., S1 stated on 3/3/2025, while in the outdoor play area, S30 allowed C1, age 4-years-old, to go into the building, into a classroom to go to the restroom independently without staff, but failed to ensure C1 returned to the outdoor play yard with her and the rest of the class. C1 was found, by O1, at approximately 4:44 p.m., alone in his classroom (not the original classroom with the restroom) without supervision when he picked C1 up for the day.
