Based on record review at 11:45 a.m., S1 failed to have a CCCBC-based determination of eligibility for child care purposes from the Department for S14 (DOH 10/01/2025) on the center’s CCCBC roster at all times. Corrective Action: Effective 12/17/2025, S1 stated she will ensure all staff have a CCCBC on the centers roaster by checking monthly to ensure compliance with this regulation.

      1911.G. Based on observations at 9:54 a.m.,S1 failed to ensure pacifiers were not attached to the infants clothing. The Specialist observed a pacifier attached, by a clip, to the clothing of C1, age 12 -months-old. S26 removed the clip during the licensing visit. Corrective Action: Effective 12/17/2025, S1 stated she will remind staff to not clip pacifiers to infants clothing to ensure compliance with this regulation.
