Based on observations, record review, and interview at 12:00 p.m., S1 failed to obtain a CCCBC-based determination of eligibility for child care purposes from the Department for S8 prior to the staff working on the premises. S8's first day present at the center was 12/17/2025, from 7:55 a.m. until 12:00 p.m. S8 vacated the premises immediately. This could not be corrected prior to the Specialist's departure.
