Based on record review on 5/20/2026, at approximately 11:30 a.m., S1 failed to have documentation that 2 of 10 staff, S7 and S9, completed mandated reporter training within 7 days from the first day present at the center and prior to assuming sole responsibilities for children. S7's first day working was 2/17/2026 and S9's first day working was 2/16/2026, and they should have completed the training on or by 2/23/2026 & 2/24/2026. Based on interviews on 5/22/2026, at approximately 10:45 a.m., S1 failed to ensure that staff had current certification in pediatric first aid and CPR within 90 calendar days from the date of hire and prior to assuming sole responsibility for any children. Based on record review on 5/20/2026, at approximately 3:30 p.m., S1 failed to obtain a CCCBC-based determination of eligibility for child care purposes from the department for each staff member, S7, prior to the person being present at the center or performing services as evidence by: S7's was present at the center on 2/17/2026, but did not receive an eligible CCCBC determination until 2/18/2026. Based on observation at approximately 1:10 p.m., children failed to be under supervision at all times as S4 walked out of her classroom with the Specialist for approximately 2 minutes to the classroom behind her classroom and left 7 two-year-old napping children alone. S4 was informed of the incident and she returned to her classroom correcting the supervision. Corrective Action: Effective 7/22/2025, S10 stated she will re-train all staff on the supervision regulation by 7/25/2025 to ensure compliance with this regulation.

1713.E..F. Based on observation at approximately 1:10 p.m., while supervising a group of children, S3, failed to devote her time to supervision of the children, during naptime. S3 was observed lying down, covered up with a blanket when the Specialist looked through the window of her classroom. S3 removed herself from the lying position prior to the Specialist leaving the center. Corrective Action: Effective 7/22/2025, S10 stated she will re-train all staff on the supervision regulation by 7/25/2025 to ensure compliance with this regulation.

1901.J. Based on observation at 1:10 p.m., items that can be harmful to children, such as cleaning supplies and chemicals failed to be kept in a locked cabinet or other secure place that ensures they are inaccessible to children. There was a generic cleaning solution in a white and blue aerosol can lying on the floor. S4 removed the cleaning supplies from the floor prior to the Specialist leaving the center. Corrective Action: Effective 7/22/2025, S10 stated they will re-train all staff on identifying items that can be harmful to children by 7/25/2025 to ensure compliance with this regulation.

1903.C. Based on observation at approximately 1:00 p.m., the indoor area failed to be free of hazards in an empty license classroom there were chairs on top of the tables which is a tipping hazard. S10 removed the chairs prior to the Specialist leaving the center. Corrective Action: Effective 7/22/2025, S10 stated they will re-train all staff on identifying and eliminating hazards in licensed space to children by 7/25/2025 to ensure compliance with this regulation.

1713. Based on observation at approximately 12:15 p.m., children failed to be under supervision at all times as when the Specialist arrived in S4's classroom she was standing in front of the cabinets facing the wall with her back turned to the 11 one to two-year-old children asleep in her classroom. The Specialist walked around the corner to identify if there was another staff in the room and there was not. S4 returned to the children prior to the Specialist leaving the room. 1901. Based on observation at approximately 12:30 p.m., the areas used by children failed to be lighted in such a way as to allow visual supervision of the children at all times. When the Specialist entered S8's classroom the lighting was too dark for the Specialist to view all of the children in the classroom. This could not be corrected during the inspection.
