Based on record review, on 5/6/2026 at approximately 11:00 a.m., S13 failed to supervise children at all times while in care at the center. On 4/30/2026 at approximately 7:46 a.m., S13 was observed on center's video with her head down, and back turned toward 5 one year old children, scrolling on a cellphone. Based on record review, on 5/6/2026 at approximately 11:30 a.m., S13 failed to follow the Electronic Devices Policy. On 4/21/2026 at 10:01 a.m., S13 was observed on center’s video sitting in a chair watching and scrolling on a cellphone while one, one-year-old child sat on her lap also viewing the contents displayed on the cellphone. Corrective Action: Effective 5/19/2026, S1 stated she has implemented a cell-phone policy, which includes: cellphones in cubbies, no longer in classrooms for the duration of staff shifts, to ensure compliance with this regulation.

Based on observations, on 5/6/2026 at approximately 9:15 a.m., S5 failed to ensure children were under supervision at all times. Upon entering the playground area, the Specialist observed 15 four to five-year-old children playing on the playground without a staff member present and supervising the group. Approximately two minutes later, S5 was observed peeking from the side of the building, making contact with the Specialist, and then assuming supervision of the children on the playground. The center did not have documentation of the following on file: CCCBC, copy of identification, information form which includes person's name, address, phone number, and list of duties performed while at the center. Based on observations, on 5/6/2026 at 9:10 a.m., S1 failed to ensure strings and cords were inaccessible to children under age 4. During the walkthrough of the center, the Specialist observed cords hanging down the wall and on the floor of a one-year-old classroom. Corrective Action: Effective 5/19/2026, S1 stated she will have staff remove any cords or strings from classrooms, to ensure compliance with this regulation.

Based on observations, on 5/6/2026 at 9:30 a.m., S1 failed to ensure outdoor area was free of hazards. During the walkthrough of the outdoor area, the Specialist observed one wooden table with two broken legs leaning against the building, sharp broken wooden pieces and exposed nails. Corrective Action: Effective 5/19/2026, S1 stated she will remove table from yard and staff will be reminded to void any broken toys or items, to ensure compliance with this regulation.

1915.B.&C.: Based on record review/interview on 5/6/2026 at 4 p.m., S1 failed to have documentation of immediate notification to the parent when the following occurred to a child: On 3/12/2026, at 6:40 a.m., C2, one-year-old, fell and hit her head on a table. The parents were not notified of the incident. Corrective Action: Effective 5/19/2026, S1 stated she has discussed procedures for incidents which include: completing reports, contacting parents immediately in regards to an incident, and within 10 minutes staff are required to contact management, to ensure compliance with this regulation.…

1919.J. Based on observations on 5/6/2026 at 9 a.m., S4 failed to have bottle formula labeled with the child's name. The Specialist observed 1 seven-month-old infant without labeled bottles. The Specialist informed S4 bottles should be labeled for all children. Corrective Action: Effective 5/19/2026, S1 stated in the mornings, prior to entering classrooms, bottles are labeled, to ensure compliance with this regulation.

: Based on record review/interview at 1:00 p.m., S1 failed to ensure all staff members completed the Mandated Reporter training annually. 1 of 11 staff members, S8 did not have current certification and expired on 07/25/2025. S1 stated each staff member will have proof of Mandated Reporter Training by 08/22/2025. Corrective Action: Effective 08/13/2025, S1 stated all staff will complete the Mandated Reporting Training immediately following their hire date and annually going forward, to ensure compliance with this regulation.

: Based on record review and interview on 03/10/2025, at 11:00 a.m., S1, Director, failed to have documentation that within 90 calendar days from the date of hire and prior to assuming sole responsibility for any children, S6 (DOH 02/17/2025) failed to have current certification in Pediatric First Aid and CPR. During the walkthrough of the center, the Specialist observed S6 alone in the classroom with 7 One-year-olds. S1 immediately put S5, Floater, in the classroom with S6.

: Based on record review/interview at 11:00 a.m., S1 failed to ensure all staff members completed the Mandated Reporter within seven calendar days of being present at the center, and prior to assuming sole responsibility for any children. S6 (DOH 02/17/2025) did not have current certification.

: Based on record review/interview at 11:00 a.m., S1 failed to have documentation of tornado drills that were conducted at least once per month during the months of March, April, May, and June. The Specialist observed there was no documentation that tornado drills had been conducted within the 2024 licensing year.
