Reviewed daily name-to-face attendance record for 4/16/26 and identified 14 children noted in attendance at approximately 2:45pm, however identified through staff statement and review of injury report only 9 children were in care at the time of injury. Observed on the date of inspection 11 children noted on the daily name-to-face record for written transitions, however only 10 children were present at the time of visit.
