Based on observations/Interview at 11:30 a.m., S1 failed to ensure medication sent to the center did not expire. The Specialist observed medicine kept at the center for C3 (3-year-old), expired on 1/30/2026, C4’s, (4-year-old) albuterol medication expired on 2/28/2026, C5’s (3-year-old) albuterol medication expired on 4/1/2025 and C6’s Epi-pen expired on 9/30/2025. Corrective Action: Effective 3/24/2026, S1 stated she will check medication monthly, to ensure compliance with this regulation.

Based on Record Review at 10:00 a.m., S1 failed to have documentation on file for Independent Contractors that included person's name, address, phone number, list of duties performed while at the center. The Specialist observed O2 was signed into the center on 3/19/2026 from 8:30 a.m. to 10:05 a.m. The center did not have the following documentation on file: CCCBC, copy of identification or the staff member’s name that accompanied the contractor at all times.
