C: Based on record review at approximately 3:30 p.m., S1 failed to notify the Department and Child Welfare within 24 hours of the following critical incident: On 3/12/2024, O1 arrived at the center with C1, 2-years-old, and informed S3 that C1 had a bruise underneath his arm that was noticed at bath time that morning and inquired if she knew what happened. S3 and S1 viewed the area identified by O1. O1 informed S1 she would take C1 for medical attention. S1 failed to report this information to Department or Child Welfare as of 5/21/2024.
