      The Provider has all six emergency forms. Two need updated information one needs an authorized pick-up and one needs Pediatrician's address. The Provider will have the families add the information and will send the LS a statement of correction.

    • LS Observed That the Hot Water Exceeded 120 Degrees13A.15.05.01E

      LS observed that the hot water exceeded 120 degrees. LS temperature reading was 132.6 F. The Provider adjust the hot water and it corrected this non-compliance on site. the temp. went down to 116 degrees.
