Identified through staff members written and verbal statements that staff members assigned the responsibility to supervise the children did not know how CH1 got injured on the playground structure. Staff member 2 stated that she had her back towards the playground area because she was helping a child, and staff 1 stated that she was at the end of the playground seated on the grass area with a child that was upset. This is a direct violation of stipulation letter C on the settlement agreement.
