    • Bathrooms

      In room 4 there was paper towels stored open under the sink, not sanitary. Room 5 had paper towels on the back of the sink, not on a dispenser and toilet paper stored open in the bathroom. Room 10 had toilet paper on top of the dispenser, none in the dispenser.

    • Poisons, Chemicals

      Poisons, chemicals, and hazardous items A bin of diaper creams, Desitin, and Aquaphor all labeled Keep out of reach of children were found accessible to children in the Zebra room under the changing table behind a child deterrent.

    • Prior to Admission

      Prior to admission, the childs parent shall establish a feeding plan, based on t 2 infants missing feeding plans prior to admission. Send documentation to consultant.

      In room 10 the diaper pail did not have a tightly covered lid that could control the odors.
