Based on record review at 12:00 p.m., S1 failed to have documentation that all staff members on the premises and accessible to children was currently certified in infant and child CPR and Adult CPR through training approved by the department. S4's training expired 3/31/2025. Corrective Action: Effective 5/22/2025, S1 stated she will have a class scheduled for S4 by 5/23/2025, and going forward, she will ensure all staff are trained at least 30 days prior to their expiration date to ensure compliance with this regulation.

During the walk through of the playground, the Specialist observed the following: a board missing from the bottom of the center building on the playground creating a crawlspace, a ladder and gutters laying on the ground outside, a shed with the roof caved in and the door missing with metal siding, boards, and screws inside of it, missing boards from the bottom of the shed on the ground, and trash inside of the shed.
