The resilient material wood chips measured less than one (1) inch in depth in the use zones around a piece of play equipment with a blue slide that measures twenty-four (24) inches in height on one side and a green slide that measures twenty (20) inches in height on the other side. This piece of play equipment was located in the outdoor play area. A red slide that measured twenty (20) inches to the platform was located on the grass in the outdoor play area and was not on any resilient material.

Learned through the interview that the provider had not given any parents/guardians an "ouch" report for minor injuries or bumps and bruises. The provider's policies and procedures state "If your child is injured and requires medical attention, you will be notified immediately. Small bumps and bruises are an everyday event for most children at some point in their lives. If you would like to be notified each time please let me know and you will be notified. Otherwise, you will be notified at the end of the day via an Ouch Report. The provider is not following their own policies for reporting injuries.
