      Based on observation at approximately 12:05 p.m., S1 failed to ensure the center was free of indoor and outdoor hazards. The Specialist observed the rolling gate located near the cafeteria/kitchen was left open allowing the children access to the parking lot when transitioning from the classroom to the cafeteria. S15 closed the gate prior to the Specialist leaving the center premises. In the cafeteria there was a gas space heater attached to the wall that was accessible to the children. This would allow the children to insert their hands and fingers into the heating elements as well…
