Based on record review and interview at 12:30 p.m., S1 failed to ensure all personnel files included the hire date and first day onsite working with children for 1 of 5 staff: S5's first day signing in was 2/4/2024, but her personnel file did not include her hire date nor first day onside working with children, only the date of the application. This was not corrected prior to the Specialist's departure.

Based on record review and interview at 12:30 p.m., S1 lacked documentation that 1 of 1 new staff, S5, received center specific orientation within seven days of the first day present at the center and prior to having sole responsibility for any children. S5, according to the sign in sheet, first day working was 2/4/2025. Center-specific orientation should have been completed by 2/11/2025. This was not corrected prior to the Specialist's departure.
