    • Bathrooms

      In the Discovery Preschool 1, Pre-K, 3's , Preschool 1 and school age restrooms paper towel was not being dispensed correctly or empty, toilet brushes were on floors or shelfs, and in preschool 1 toilet paper was sitting on back of toilet instead of in dispenser.

    • Caregivers Shall Use the Diapering Surface Exclusively for Diaper Changing

      In the Todd 1 room there was painting smocks on the changing table. In DPS 1 and 2 there were supplies being stored on the changing table.

    • Bathrooms

      In DPS 1, PK, PS 2, SA rooms and the kitchen had paper towels sitting on counters, not on dispensers, or stored openly in the bathroom, not sanitary. DPS 2 did not have toilet paper available at the toilet. PK toilet paper dispenser is broken.

    • The Center Shall Serve Breakfast

      The center shall serve breakfast, a morning snack, lunch, and a mid-afternoon sn Center is not serving a morning snack daily and menus do not state morning snack is an option.

    • Kitchen and Food Preparation Areas

      At the time of inspection there was no thermometer found in the freezer closest to the wall and the other freezer temperature was 20 degrees F.
