Based on record review and interview at 1:00 p.m., S1 failed to provide documentation that 1 of 4 staff, completed the LDE Key Orientation Training Module 3. S2 (date of hire 08/01/2023) did not complete the training within 30 calendar days of her first day present at the center and prior to assuming sole responsibility for any children. The Specialist observed S1 working in the classroom with three children, aged one to five-years-old. Corrective Action: Effective 6/10/2026, S1 stated S2 would complete the training immediately to ensure compliance with this regulation.

Based on observations at 1:00 p.m., S1 failed to ensure that a blanket for covering the child was provided by either the center or the parent. The Specialist observed 13 children resting during naptime without a blanket covering them. Corrective Action: Effective 06/10/2026, S1 stated she will request that parents bring blankets for each child to ensure compliance with this regulation.

1509.A.8.a&b.: Based on record interview at 10:45 a.m., although the center has a behavior management policy, S2 used a prohibited method of discipline. On 10/10/2025, S2 threatened to punish C1, 3-year-old, by popping him on his hand with a stick. Corrective Action: Effective 10/23/2025, S1 stated that S2 will attend a Conscious Discipline course and will retrain staff on the center's Behavior Management Policy to ensure compliance with this regulation.

1727.A B.: Based on the record review and interview of staff files on 10/23/2025 at 10:30 a.m., S1 failed to have documentation that 2 of 3 staff members had completed the online child abuse and neglect Mandated Reporter Training provided by DCFS annually. S1 has no record on file, and S2's last Mandated Reporter was completed on 9/24/2024. S1 provided training certificates, in which S1 and S2’s names were fraudulently added to the document by S1, not by the organization that provided the training. Based on observations, at 10:15 a.m.., S2 failed to supervise children at all times in the center, The Specialist observed C1, four-year-old, out of S2's sight. S2 did not provide adequate supervision for C1 while the child ran down the hallway into the back Infant classroom, which was empty. The Specialist observed C1, run back down the hallway into the 3-4 year old classroom. Corrective Action: Effective 7/1/2025, S1 stated she will speak with C1's mother, due to her Autism, of ways to address her behaviors while at the center, to ensure compliance with this regulation.

Based on record review/interview at 11:00 a.m., S1 failed to provide documentation of End-of-Day-Check. Results including date and signature of Staff member completing check has not been documented. The Specialist observed that the End-of Day-Check had not been completed for January 2, 3, 6-10, 13-15, of 2025, February 3-7 of 2025, March 20, 27 and 30th of 2025, April 1, 2, 7, 22, 29, and 30th of 2025, May 7, 2025, and June 26-27, 2025. 1901. M.: Based on observation at 10:30 a.m., S1 failed to have strings and cords inaccessible to children under the age 4. During the walk-through of the center, the Specialist observed several cords coming from the air conditioner units in the infant and toddler classrooms. Corrective Action: Effective 7/1/2025, S1 stated she will find cord covers to cover the exposed cords, to ensure compliance with this regulation.

Based on observation/record review/interview on 1/10/2025, at 11:25 a.m., S1 failed to maintain accurate and complete daily attendance records for the Director. The Specialist observed that S1 had not signed in or out from 9/3/2024 to 12/2/2024 and 12/9/2024 to 1/10/2025.

Based on record review at 10:50 a.m., S1 failed to provide documentation of observations noting when something is observed, noted on children, upon arrival to the center. Results including an explanation from parent and/or child were not documented. Daily observations logs had not been completed and there were no records to review.

1919.J. Based on observations/interviews on 1/10/2025 at 10:55 a.m., S2 did not have bottle formula/breast milk for infants labeled with the child's name. The Specialist observed 1 infant, 11-months-old, without labeled bottles. The Specialist informed S1 and S2 that bottles should be labeled for all children.
