      LS observed a sub on site but no sub log. LS discussed this regulation with the director when she arrived at the program. The director states she will complete a sub log moving forward. Program is to send proof of a current sub log to OCC ASAP.

      LS observed 2 staff on site and neither has the MA training. The director states she has been trying to find a MA class for quite some time but the classes are either extremely far away or offered on Sundays. LS provided the director with a few more names on the MA trainers list from the website as
