(1) A provider shall maintain: (a) A current immunization certificate for each child in care within thirty (30) days of the child’s enrollment, unless an attending physician or the child's parent objects to the immunization of the child pursuant to KRS 214.034; Findings: General: Based on interview and review of documentation, two children’s enrollment files (DOE: 06/17/2024 and DOE: 10/01/2025) did not contain a current immunization certificate.

922 KAR 2:100 - Section 13. Care Requirements for a Provider. (1) A provider shall ensure the health, safety, and comfort of each child. Findings: General: Based on observation, a loose baby wipe in the front programming space was accessible to the children. A one (1) year-old child was observed carrying the wipe about while placing it in their mouth and chewing on it.
