      Per collateral contacts, one staff was observed roughly sitting a child down, roughly grabbed children by the shoulders, pushed a child against a door and pulling on a child's arm. A separate staff was observed lifting a child up by one arm in a rough manner and roughly sitting a child down.

      Staff did not demonstrate sound judgement or an understanding of a child in their care; a 2.5 year old child went into the bathroom and removed his pullup, sat on the toilet and then proceeded to sit on the floor, where he urinated.
