Spoons were not provided for children in Room 1, Room 5, Room 6 and Room 7. Children had a chicken and rice bowl for lunch. Children were not provided plates in Room 4 and the fruit was placed on a paper towel and became soaked with the juice from the fruit.

Per staff statement, children do not brush their teeth daily after meals.

In Room 2, there was open popcorn and in Room 5 there was open cheddar snacks that were not stored in containers with a tight fitting lid.

Per staff statement, seconds of milk are not available.

Staff to child ratio was not maintained in several units. In Room 6. licensed for children 2 1/2 years to school age or 3 years to school age, when there is one staff with 14 children. In Room 2 licensed for children 2 weeks to 12 months or 2 weeks to six years, when there were 12 infants with three staff. Room 4, licensed for 10 children 12 months to 2 1/2 years, had 11 children with 2 staff. Room 1, licensed for 2 to 3 years old, had 10 children with 1 staff.

Terms of the license was not followed in Room 2 when there were 12 infants in the unit and the until is licensed for nine infants. In Room 4, licensed for 10 children, there were 11 children present.

The stairs leading to the basement are not guarded.

Premises is not maintained in good condition when the door in Room 5 that leads directly outside will not open easily.

Equipment is not in good repair in Room 4 when the hinge is broken on the cabinet door where diapers are stored.
