      At time of visit, consultant observed a two (2) month old female sleeping in a bassinet with two blankets, a boppy pillow and a bottle.

      A provider that cares for not more than sixteen (16) children at a facility where the provider operates a child care program shall maintain the same group sizes as specified in the group size requirements; that apply to a child care home under IC 12-17.2-5.

      At time of visit, no certificate was available to verify safe sleep training course for director.

      Each child has age appropriate immunizations including Varicella and Pneumococcal vaccines. Documentation includes: -Attendance records of all children in attendance. Immunization records for each child (includes month, day and year given for each immunization and childs birth date).
