591-1-1-.03(1) requires the Center to provide a daily planned program of varied and developmentally appropriate activities to promote social, emotional, physical, cognitive, language and literacy growth. Center Staff shall use a variety of teaching methods to accommodate the needs of the children’s different learning styles. It was determined based on obsevation that the lesson plans in Rooms A, C

591-1-1-.14(2)(b) requires that there must always be one staff person present in each classroom where children are present that has current and valid pediatric cardiopulmonary resuscitation (CPR) and pediatric first aid training. It was determined based on review of records that staff person #6 did not have evidence of current and valid pediatrick CPR and first aid training on file.

591-1-1-.25(13) requires that potentially hazardous equipment, materials and supplies be stored in a locked area inaccessible to children. It was determined based on observation that a a teacher's purse was stored on cots and baby wipes were stored on a shelf in the bathroom in Room A. It was further determined that a pair of adult scissors was stored in an unlocked cabinet in Room D.

591-1-1-.26(9) requires the playground to be kept clean, free from litter and free of hazards, such as but not limited to rocks, exposed tree roots and exposed sharp edges of concrete. It was determined based on observation that there were three PVC border pipes that were detached along the bottom of the fence which posed a potential tripping or entrapment hazard.

591-1-1-.25(13) requires that potentially hazardous equipment, materials and supplies be stored in a locked area inaccessible to children. It was determined based on observation that a box of rubber gloves and empty plastic garbage bags were stored in an unlocked drawer in Room A.
