      There was 1 out 3 that had an expired annual Mantoux tuberculin test or chest x-ray for direct child care providers and all family members over eighteen (18) years of age.

    • Caregiver Shall Keep Poisonous or Hazardous Materials That Would Harm Children,

      In the back room where the changing table on the lower shelf there was destin cream and other cream that was in reach of children that all states keep out of reach of children. In the office the door was open and the staff purse/bag was on the floor that is within reach of children.
