Reviewed nine (9) staff files. The staff file for one (1) staff member did not contain documentation that the staff member has completed a current "Child Abuse Prevention" training. The specialist also searched in PDIS for verification of the training. The staff member has been employed since August of 2025.

The wood chip resilient material was frozen and unable to be measured. At the time of inspection three (3) children were observed playing on the climbing play structure and slide that are over eighteen (18) inches in height.

At the time of inspection the wood chip resilient material measured a depth of three (3) inches in the use zone under the monkey bar section of the play structure in the outdoor play area.
