      One child is missing an emergency form, one needs the parent to sign the form, one needs the date on it, and one needs the emergency contacts' addresses added. Provider will send the 4 corrected emergency forms to LS for correction.

      When LS arrived at the home, provider had 3 children under the age of 2 in care. Shortly after LS arrived, a parent came to pick up one of the children, so provider was then in compliance. Some attendance records were reviewed and there was another day where it looked like there had been 3 children

      When LS arrived, there was air freshener and Febreeze within reach of children. Provider moved it but this will be checked upon follow up. LS discussed the use of air fresheners and diffusers with provider as well.
