1713.B. Based on record review/interviews at 10:13 a.m., S3 failed to ensure that children were supervised at all times. On 10/31/2025, at 9:31 a.m. S3 was in her classroom with 5 children, age 2-years-old. S4 stood at the classroom’s door with her back turned to the children for approximately 2 minutes. S4 then left out of the classroom with C3, leaving 3 children left unattended for approximately 1 minute. Based on record review/interviews, at 9:44 a.m., although the center had a written behavior management policy implemented, S3 (DOH 10/07/2024) used a prohibited method of discipline as C1, age two-years-old, was subject to physical/corporal punishment. On 10/31/2025 at 8:40 a.m., S3 hit C1 on her left hand because C1 was “undoing” everything S3 cleaned up in the classroom. 1903.C. Based on observation/interviews at 11:26 a.m., S1 failed to have the indoor area free of hazards.. On 10/31/2025, C2, age 2-years-old put two bottles of glue in his mouth while S4’s back was turned. The directions on the glue bottle indicates “Choking Hazard, small parts not for children under 3-years-old.” Corrective Action: Effective 11/17/2025, S1 stated she will remind staff while doing activities with the children that entails using school glue, to remove the glue and place in the cabinet where it is out of reach of the children to ensure compliance with this regulation.

Based on record review/interviews on 12:22 p.m., S3 failed to lift children using proper lifting techniques as on 10/31/2025, S2 stated S3 was observed, on the center's video footage, lifting C2 age 2-years-old, by one arm (left arm) and carry him from one side of the classroom to the other side of the classroom. Corrective Action: Effective 11/17/25, S1 stated she will conducting monthly trainings with all staff about proper lifting techniques to ensure compliance with the regulation.

Based on interview and record review at 12:45 p.m., S1 failed to immediately notify the Parent of the following critical incident: On 9/8/2025 it was reported, S25, former three-years-old Teacher, was witnessed hitting C1, age three-years-old, on the head with a cup, pinch him and grab his fingers in a forceful manner. Based on observation at 1:27 p.m., S1 failed to have the outdoor play space enclosed with a permanent fence or other permanent barrier in a manner that protects children from traffic hazards, prevents children from leaving the premises without proper supervision, and prevents contact with animals or unauthorized persons. The Specialists observed 2 fence boards around play yard 3 damaged with missing pieces, and 4 boards missing from a barrier that encloses hazardous items. Corrective Action:

The parent was notified at 12:41 p.m. On 6/9/2025, at 8:30 a.m., a child (age unknown), slipped and hit her head. The parent was notified at 3:46 p.p., during pick-up. Based on interview on 6/20/2025, at 12:45 p.m., although a mandated reporter, S17 did not report an allegation or suspicion of abuse/neglect of a child to the Child Protection Statewide Hotline as on 6/17/2025, at approximately 9:45 a.m., S17 witnessed S9 grab a child by the wrist. S17 stated S9 was on her cellphone and told C2, 3-years-old, to go play. C2 replied no and S17 grabbed and pulled C2s wrist and told her to go play.
