Minimum staff-child ratio was exceeded at the time of the survey. At the onset of the survey, the sole Room 107 staff was present outdoors with 18 children (ages infant- 5 years of age) resulting in facility over ratio by 12 children and over capacity for the unit by 9 children.- This was corrected at the time of the survey when the two staff (assigned to the other two units of the facility) arrived in the outdoor play area.

Method of behavior management used by staff did not support the child in developing inner controls and managing acceptable social behavior as evidenced by a staff member grabbing a 3-year-old child by the arm who was standing on a table which resulted in the child getting a scratch on their arm.

Ten toddler age children in Room 106 were unattended when sole staff of unit left to answer the door.

Minimum staff-child ratio was exceeded on 3/21/2025. On the onset of the survey, ten toddler aged children were in Room 106 with one staff person, causing the ratio to be exceeded by four children. Then, Room 106 combined with Room 109 for outdoor play. The sole Room 109 staff was present outdoors with sixteen children (aged toddler- 5 years of age) resulting in facility over ratio by ten children.

Facility operated outside the age specified within the license for Room 109 by having children under the age of 2.5 years present; observed by licensing specialist on 3/21/2025 and through provided photographic evidence.

Sole staff present in outdoor play area was not actively engaged in the supervision of all children in his care 3/21/2025 on the playground. Licensing specialist observed concerning behaviors among children that staff was unaware of until notified by the licensing specialist,even though a child was crying.

Toddlers were combined with older children, observed by licensing specialist on 3/21/2025 for playground time.

Confidentiality of child's information was not maintained. Assistant director was overheard discussing personal and behavioral information about children to a couple touring the facility and again to a different parent, unrelated to the child.

No child files were maintained or available for review for the twenty children in care at the time of the survey.
