S3 was the only staff member with current training in medication administration. Corrective Action: Effective 2/26/2026, S4 stated she will have all staff complete the required medication administration training to ensure compliance with this regulation.

Based on observations, record review, and interview on 2/26/2026, S1 failed to ensure that a CCCBC-based determination of eligibility for child care purposes from the department was obtained for each staff member, prior to the person being present at the center or performing services as evidence by: S4 was observed working at the center completing administrative duties and assisting in the classrooms, and S2 was observed supervising a classroom of 7 children, aged 2-4 years-old at the time of the inspection. Based on observations and interview on 2/26/2026, S1 failed to document that the entire center and play yard is checked after the last child departs to ensure that no child is left unattended at the center. There was no documentation of an end-of-day check from 1/21/2026 to 2/25/2026. S4 stated she was in the process of printing forms to be completed each day. Corrective Action: Effective 2/26/2026, S4 stated she will print the required forms which will be completed each day to ensure compliance with this regulation.
