      Based on record review and interviews at 10:30 a.m., S1 failed to notify the parents immediately and the Department and Child Welfare within 24 hours of being made aware of the following critical incident: On 11/12/2025, S5 informed S1 that S6 stated to her that she had video recordings of her stepping on a male child's hand and pinching and kicking a female child in the back (date unknown).
