      Based on record review at approximately 12:45 p.m., S3 and S4 failed to have a daily written or electronic report for 1 of 1 infant, C1, 4-months-old. S3 and S4 created and updated the report prior the Specialist leaving the center. Corrective Action: Effective 3/27/2026, S1 stated she will review infant reports daily at 9:00 a.m., to ensure that they are completed and again at 1 p.m. to ensure that they are up-to-date to ensure compliance with this regulation.

      Based on record review at approximately 12:35 p.m., S1 failed to have documentation of a current annual inspection and approval from State Fire Marshal. The date of the last approval is 1/6/2025. S1 stated State Fire Marshal stated their inspection was current. This could not be corrected during the inspection.       Based on record review at approximately 11:50 a.m., the center's staff and owner's daily attendance record failed to accurately reflect persons on the child care premises at any given time as S1 was documented as signed into the center at 7:45 a.m. but failed to be on the premises. S5 stated S1 left the center at 8:30 a.m. This could not be corrected during the inspection.
