Based on record review/interviews at 12:45 p.m., S3 failed to ensure children were under supervision at all times. On 07/21/2025 at 11:00 a.m., C1, age twoyears-old, was in the indoor play area during transition and ran out of the exit door towards the outside play yard. C1 was unsupervised for approximately 1 minute. C1 was found by S6 and given to S7 to return to S3 in the classroom. Corrective Action: Effective 07/22/2025 S1 stated she will retrain all staff to actively engage and supervise children at all to ensure compliance with this regulation.
