The program director of the facility did not report to DCF evidence of suspected child abuse within 24 hours. The program director was provided evidence end of day 3-20-24. Report made to DCF end of day 3-22-24.

Documentation of collateral contacts indicate that on 3-20-24 a staff member removed a five-year-old child from an outdoor play structure by the arm, yanked the child up from a seated position by the arm, and forcibly restricted the child's movement by holding her by the hand and wrapping her arms around the child as the child resisted.

I want it quiet. Don't talk."

Multiple cots in the preschool room did not have a bottom sheet while in use. One cot in the middle room and two cots in the west room did not have bottom sheets while in use.

One 11 month old infant was napping in the crib with one lose blanket and an additional crib sheet bunched up in his hands and around his face as a comfort item. *The soft items were removed at visit per surveyor request.
