      The center must store cleaning equipment, cleaning agents, aerosol cans, and any Items labeled 'Keep out of reach of children' were accessible in Room 7/School Age: Quat, bleach, Resolve in an unlocked upper cabinet.

    • The Center Must Seal All Walls and Floors in Restrooms

      Bathroom in Room 7 had peeling paint on wall. Bathroom in Room 8 had flooring that appeared to be bubbled up. Bathroom in Room 11 had standing water on floor due to a leaking toilet and second bathroom had a crack in flooring.
