1711.A.B.D. Based on interviews at 12:15 p.m., S1 failed to meet the required child-to-staff ratio on 8/28/2025, at 4:43 p.m., for 9 children, ages 3-years-old through 10-years-old. S8 and S9, who are both 17-years-old and had sole responsibility of the children, failed to be under the direct supervision of a qualified adult staff. A qualified adult staff was required to be present to meet the child-to-staff ratio. Corrective Action: Effective 9/17/2025, S1 will pair each staff member under age 18 with a qualified adult staff member each day to ensure compliance with this regulation.

Based on record review at 10 a.m., S8 (DOH: 8/7/2025) and S9 (DOH: 8/7/2025) failed to have current certification in pediatric first aid and CPR within 90 calendar days from the date of hire and prior to assuming sole responsibility for any children. On 8/28/2025, at 4:43 p.m., S8 and S9 had sole responsibility for 9 children, ages 3-years-old through 10-years-old, while on the play yard. Corrective Action: Effective 9/17/2025, S1 will have all new staff trained for CPR and First Aid prior to assuming sole responsiblity to ensure compliance with this regulation.
