Per staff statement, children in the Honeybee unit and the Galaxy units were put on nap cots at noon and at 1:20, children who were not napping remained on their cots with no quiet activities.

Cots in the Galaxy unit were not at least two feet apart.

One unit does not have a person with training in CPR and First Aid.

Per staff statements, and licensing specialists' observation, children who are not napping are not given quiet activities.

One unit did not have a lead teacher with the required qualifications.

One child in care does not have documentation of current immunizations on file.

Two of 12 children's files reviewed do not have a complete emergency medical treatment form on file.

Terms of the license is not being followed in the Honeybee unit, for 11 children 2 1/2 to school age, when there are two children, both 2 years and 5 months old in the unit.

Cleaning supplies, air freshener and laundry soap are unlocked and accessible to the children in the Galaxy unit and Honeybee unit. The cabinet was unlocked under the kitchen sink with Raid multi-insect spray, bleach, dishwasher pods and other products with warning labels and the children have access through the kitchen.

The kitchen is not cleaned and free from evidence of vermin or rodent infestation. Small bugs, appearing to be roaches, were observed under the sink in the kitchen. There are roach traps, with dead roaches in them, and an unknown, white powdery substance, around the edges of the counters and on the floor.

Two outlets in the Galaxy unit are uncovered and accessible to children.

The Honeybee and Galaxy units did not have written lesson plans available at the time of the survey.

Two units, Honeybee and Galaxy, do not have a sufficient quantity of toys to give the children a choice of at least three activities.

Utensil was not washed and sanitized when a child dropped the spoon on the floor, staff rinsed it off in the bathroom handwashing sink and gave it back to the child.

Open cereal in the kitchen was not stored in a sealable plastic bag or airtight container.

Sanitary methods of food handling are not practiced when a common hand towel is used in the kitchen and washed two to three times a week, per staff statement.

The following diapering procedures were not followed in the Honeybee room: #7 and #8 Staff did not wipe their hands with a moist toilette prior to dressing the children (gloves were not used); #9 Staff did not wash the children hands; #11 Staff did not clean and disinfect the diapering area; and #12 Staff did not wash their own hands.

In the Honeybee unit, licensed for 2 1/2 years to school-age, children were not diapered on a changing table or non-porous pad when the children were laid on the floor, and when the paper towels were removed, the child bottom was on the bare floor.

In the Toy Story Room children did not wash their hands before the noon meal.

Tables were not washed before the noon meal in the Toy Story Room.

Children were not encouraged to serve themselves in the Honeybee Room and the Toy Story Room when staff served each meal component.

One child, DOB 9/21/17, was left unsupervised in the Red Room upon surveyor's arrival.

One child was not given a cot to sleep on. Child was asleep on the floor.
