Based on record review and interviews at 10:15 a.m., S5 failed to document observations, when something is observed, noted on children upon arrival to the center. On 04/28/2026, at approximately 3 p.m., C1, eight-months-old, busted her lip while at the center. S5 stated C1 had a blood clot on her lip upon arrival on 04/29/2026, however, it was not documented. Corrective Action: Effective 05/13/2026, S1 stated, going forward, all staff will document anything observed on children upon arrival to the center, even if it happened at the center, to ensure compliance with this regulation.

1915.B.&C.: Based on record review and interviews at 10:15 a.m., S1 failed to have documentation of immediate notification to the parent when the following occurred to C1, eight-months-old. On 04/28/2026, at approximately 3 p.m., C1 was sitting in her car seat, not properly strapped in, when she sat up and fell out causing her to bust her top lip. O1, parent of C1, was notified upon arrival to the center at 3:10 p.m., however, S5 did not complete an incident form.
