This could not be corrected. Corrective Action: Effective 4/17/2026, S1 stated she would keep a record of the daily passenger logs on the bus and at the center. She will have a meeting with the drivers on 4/17/2026 about protocol to ensure compliance with this regulation.

Based on observations and interview at 10:15 a.m., S1 failed to ensure the Director's duties consisted only of administrative duties when the number of children in care exceeded 42 as there were 44 children at the center and S1 was observed supervising 25 5-to-9-year-old school-aged children with S2. This was corrected when S7 arrived. Corrective Action: Effective 2/25/2026, S1 stated she will ensure enough staff are scheduled when 42 or more children are present to ensure compliance with this regulation.
