      Poisons, chemicals, and hazardous items In the Discovery Preschool 1 room, a Poisons, chemicals, (gallon of bleach )was stored under the changing table unlocked.

    • Child/staff Ratios and Supervision

      Ratio was not maintained during nap time when the toddler class combined with the Discovery pre school class with 7 children ages 1 and 2 with 1 qualified caregiver. Naptime ratio was not observed as 6 or the 7 children were awake.

      Discovery Preschool 1 room: Area rug next to the changing table, not allowing a 2 foot sanitizable area around the changing table.

    • Bathrooms

      Toddler 2 room: bathroom toilet paper stored on top of the dispenser, and no toilet paper in the dispenser. Discovery Preschool 1 room: bathroom toilet paper stored on top of the dispenser, and no toilet paper in the dispenser.
