      A stairwell with a drop-off of more than 24 inches had a railing/barrier was not constructed safely as evidenced by the wires were exposed causing potential cutting hazard and the rail was not tight allowing for a fall.

      The walls located door trim in blue room and both vanities in the bathrooms and trim around the closet with cubbies was/were not in good condition as evidenced by broken or missing.

      There was evidence of dirt/debris in in the vents above the door way into the hall and vents into the furnace room.

      The facility did not have a fenced outdoor play area that was on or adjoining the day care property and was accessible without hazard as evidenced by the fence was not in good condition in that fence fasteners were missing on several posts allowing the fence not to be secured .
