Based on record review at 12 p.m., S1 failed to ensure S5 completed the DCFS online Mandated Reporter Training within 7 calendar days of the first day present at the center on 9/23/2025. The training was to be completed no later than 9/29/2025, and S5 completed the training on 10/2/2025. Corrective Action: Effective 10/28/2025, S1 stated she will have all new hire staff complete the training prior to their first day present in the center, to ensure compliance with this regulation.

Based on observation at 10:30 a.m., S1 failed to ensure S3 had a current a certification in pediatric first aid and S5 had a current certification in pediatric first aid and CPR within 90 calendar days from the date of hire and prior to assuming sole responsibility for children. S3's date of hire is 8/22/2025 and had sole responsibility for 8 children, aged 2 to 4-years-old and S5's date of hire is 9/22/2025, and had sole responsibility for 5 children, aged 1-year-old. Based on observation at 10:35 a.m., S1 failed to ensure a bottle warming device was inaccessible to children. The Specialist observed an electric bottle warmer on the edge of a waist-level shelf in the infant classroom. S1 removed the bottle warmer prior to the Specialist leaving the classroom. Corrective Action: Effective 10/28/2025, S1 stated she will keep the bottle warming device in the kitchen to heat all infant bottles, to ensure compliance with this regulation.

Based on observation and interview at 10:49 a.m., S1 failed to ensure the outdoor area was free of hazards as the Specialist observed the plastic benches on the picnic table in the play yard were cracked and broken, with two exposed screws. S1 stated she will remove the bench no later than the close of business. Corrective Action: Effective 10/28/2025, S1 stated she will complete a visual walk-through of the center and play yard daily and remove all safety hazards, to ensure compliance with this regulation.
